This is a ‘SINGLE PROBLEM FOCUSED EXAM’. The doctor will address your ‘Chief Complaint’ by taking a medical history, listening to your testimony, completing a thorough examination as it relates to your problem and performing diagnosis, prognosis and treatment planning within our scope of treatment. Necessary referrals and or Rx will be provided depending on the need and treatment can be done the same day, if possible.

*Usual and customary fee is $278. For new patients only and one use per person lifetime.  Cannot be combined or used with dental insurance. Those with dental insurance we are contracted to are restricted to dental insurance. Applies where full fee applicable (Non-insurance). ‘Cleaning’ constitutes Dental Prophylaxis in absence of periodontal disease (D1110). Comprehensive exam is D0150.
